Health & safety
Medical forms
A medical team is on site at every rehearsal and performance. These forms are what let them help your student safely. Questions go to LISD Health Services.
Off-campus medical consent
Required if any prescription or over-the-counter medication may be needed at rehearsals, games, or performances. Print double-sided.

Required for any student who carries their own medication, such as an epi-pen, inhaler, Diastat, or glucagon.
